Transcript Request

This form can be submitted as many times as needed to request transcripts for post secondary institutions, military, employers, or scholarship applications.  Contact Melissa Zoelle with questions at mzoelle@kiel.k12.wi.us or 920-894-2266 ext 2507. (NOTE:  The Permission to Release Records form must be received prior to transcripts being released to other institutions.  The form is available HERE.  It only needs to be completed one time).
